Resumo Fundamento: O prolongamento do intervalo PQ, geralmente associado a um atraso na condução atrioventricular, pode estar relacionado a alterações na propagação do impulso intraventricular. Objetivo: Avaliar, por meio do mapeamento do potencial de superfície corporal (BSPM), o processo de despolarização ventricular em atletas com intervalos PQ prolongados em repouso e após o exercício. Métodos: O estudo incluiu 7 esquiadores cross-country com intervalo PQ superior a 200 ms (grupo PQ Prolongado) e 7 com intervalo PQ inferior a 200 ms (grupo PQ Normal). O BSPM de 64 derivações unipolares do tronco foi realizado antes (Pré-Ex) e após o teste ergométrico de bicicleta (Pós-Ex). Mapas equipotenciais da superfície corporal foram analisados durante a despolarização ventricular. O nível de significância foi de 5%. Resultados: Comparado com atletas com PQ Normal, o primeiro e o segundo períodos de posição estável dos potenciais cardíacos na superfície do tronco foram mais longos, e a formação da distribuição de potencial "sela" ocorreu mais tarde, no Pré-Ex, nos atletas com PQ Prolongado. No Pós-Ex, o grupo PQ Prolongado apresentou um encurtamento do primeiro e segundo períodos de distribuições de potencial estáveis e uma diminuição no tempo de aparecimento do fenômeno "sela" em relação ao Pré-Ex (para valores próximos aos do Normal -Grupo PQ). Além disso, no Pós-Ex, a primeira inversão das distribuições de potencial e a duração total da despolarização ventricular em atletas com PQ Prolongado diminuíram em comparação com o Pré-Ex e com valores semelhantes em atletas com PQ Normal. Em comparação com atletas com PQ Normal, a segunda inversão foi mais longa no Pré-Ex e Pós-Ex em atletas com PQ Prolongado. Conclusão: Atletas com PQ prolongado apresentaram diferenças significativas nas características temporais do BSPM durante a despolarização ventricular, tanto em repouso quanto após o exercício, em comparação com atletas com PQ normal.


Abstract Background: Prolongation of the PQ interval, generally associated with an atrioventricular conduction delay, may be related to changes in intraventricular impulse spreading. Objective: To assess, using body surface potential mapping (BSPM), the process of ventricular depolarization in athletes with prolonged PQ intervals at rest and after exercise. Methods: The study included 7 cross-country skiers with a PQ interval of more than 200 ms (Prolonged-PQ group) and 7 with a PQ interval of less than 200 ms (Normal-PQ group). The BSPM from 64 unipolar torso leads was performed before (Pre-Ex) and after the bicycle exercise test (Post-Ex). Body surface equipotential maps were analyzed during ventricular depolarization. The significance level was 5%. Results: Compared to Normal-PQ athletes, the first and second periods of the stable position of cardiac potentials on the torso surface were longer, and the formation of the "saddle" potential distribution occurred later, at Pre-Ex, in Prolonged-PQ athletes. At Post-Ex, the Prolonged-PQ group showed a shortening of the first and second periods of stable potential distributions and a decrease in appearance time of the "saddle" phenomenon relative to Pre-Ex (to the values near to those of the Normal-PQ group). Additionally, at Post-Ex, the first inversion of potential distributions and the total duration of ventricular depolarization in Prolonged-PQ athletes decreased compared to Pre-Ex and with similar values in Normal-PQ athletes. Compared to Normal-PQ athletes, the second inversion was longer at Pre-Ex and Post-Ex in Prolonged-PQ athletes. Conclusion: Prolonged-PQ athletes had significant differences in the temporal characteristics of BSPM during ventricular depolarization both at rest and after exercise as compared to Normal-PQ athletes.

ABSTRACT Objective: To analyze spatial distribution of preterm births and their association with maternal, social, and health services indicators in the metropolitan region of São Paulo, Brazil, 2010-2019. Methods: Ecological study using data on preterm newborns from 39 municipalities in the metropolitan region of São Paulo. Univariate global Moran's index (Im) was used to evaluate spatial association of prematurity, and univariate local Moran's index by using the cluster map (LISA) to identify spatial patterns and clusters. Bivariate global Moran's index was also used to analyze spatial autocorrelation with maternal, social, and health services indicators. Results: A total of 3,103,898 live births were registered in period 2010-2019, of which 331,174 (10.7%) were preterm. The global Moran's index showed spatial independence (Im=0.05; p-value=0.233) of the proportion of preterm births between municipalities. However, in the local spatial analysis it was possible to identify a statistically significant spatial cluster between the municipalities of Biritiba Mirim, Guararema and Salesópolis, with high proportions of preterm births. In the bivariate analysis, a significant positive spatial association was identified with proportions of mothers under 20 years old (Im=0.17; p-value=0.024) and mothers with low schooling (Im=0.17; p-value=0.020), and a significant negative spatial association with HDI (Im=-0.14; p-value=0.039). Conclusions: The local spatial approach identified a spatial cluster located in the far east of the metropolitan region of São Paulo, where actions by health managers are needed to minimize occurrence of preterm births.


RESUMO Objetivo: Analisar a distribuição espacial dos nascimentos prematuros e sua associação com indicadores maternos, sociais e de serviços de saúde na região metropolitana de São Paulo, Brasil, 2010-2019. Métodos: Estudo ecológico utilizando dados sobre recém-nascidos pré-termo dos 39 municípios da região metropolitana de São Paulo. Utilizou-se o índice de Moran (Im) global univariado para avaliar a associação espacial da prematuridade, e o índice de Moran local univariado por meio do mapa de clusters (LISA) para a identificação de padrões e aglomerados espaciais. Também foi utilizado o índice de Moran global bivariado para analisar a autocorrelação espacial com os indicadores maternos, sociais e de serviços de saúde. Resultados: Foram registrados 3.103.898 nascidos vivos no período 2010-2019, dos quais 331.174 (10,7%) foram prematuros. O índice de Moran global mostrou independência espacial (Im=0,05; p-valor=0,233) da proporção dos nascimentos prematuros entre municípios. No entanto, na análise espacial local foi possível identificar aglomerado espacial estatisticamente significativo entre os municípios de Biritiba Mirim, Guararema e Salesópolis, com proporções altas de nascimentos pré-termo. Na análise bivariada, identificou-se associação espacial significativa positiva com proporções de mães menores de 20 anos (Im=0,17; p-valor=0,024) e mães com baixa escolaridade (Im=0,17; p-valor=0,020), e associação espacial significativa negativa com IDH (Im=-0,14; p-valor=0,039). Conclusão: A abordagem espacial local identificou agrupamento espacial situado no extremo leste da região metropolitana de São Paulo, onde ações dos gestores de saúde são necessárias para minimizar a ocorrência de partos prematuros.

Abstract Objective: To analyze the temporal trend and the spatial distribution of acquired syphilis in Mato Grosso, Brazil, between 2010 and 2021. Methods: This was an ecological study using notifications of acquired syphilis held on the Notifiable Health Conditions Information System. Detection rates were calculated by health macro-region and three-year periods (2010-2012, 2013-2015, 2016-2018, 2019-2021). The jointpoint method was used to calculate annual percentage change (APC). Thematic maps of Bayesian rates were built and distribution was analyzed using Local Moran. Results: The detection rate increased from 16.2 per 100,000 inhabitants in the first three-year period (2010-2012) to 70.0 in the last three-year period (2019-2021). The Central-North macro-region had the highest rate in the last three years (94.3/100,000 inhab.), while the highest upward trend occurred in the Central-Northwest macro-region, from 2013 to 2018 (APC = 50.2; 95%CI 26.3;78.6). There was an increase in Bayesian rates in most municipalities. Conclusion: There was a trend towards an increase in acquired syphilis, especially in the last two three-year periods.


Resumen Objetivo: Analizar la tendencia temporal y la distribución espacial de la sífilis adquirida en Mato Grosso, de 2010 a 2021. Métodos: Estudio ecológico utilizando informes de sífilis adquirida del Sistema de Información de Enfermedades de Declaración Obligatoria - Sinan. Las tasas de detección se calcularon por macrorregión de salud y trienios (2010-2012, 2013-2015, 2016-2018, 2019-2021). Se utilizó el método jointpoint para calcular el cambio porcentual anual (CPA). Se construyeron mapas temáticos de tasas bayesianas y se analizó la distribución por Moran Local. Resultados: La tasa de detección aumentó de 16,2 por 100.000 en el primer trienio (2010-2012) a 70,0 en el último trienio (2019-2021). La macrorregión Centro-Norte presentó la tasa más alta en los últimos tres años (94,3/100.000 hab.), mientras que la mayor tendencia al alza se presentó en la macrorregión Centro-Noroeste, de 2013 a 2018 (CPA = 50,2; IC95% 26,3; 78,6). Hubo un aumento en las tasas bayesianas en la mayoría de los municipios. Conclusión: Hubo una tendencia al aumento de la sífilis adquirida, especialmente en los dos últimos trienios.

A macrorregião Centro-Norte apresentou a maior taxa no último triênio (94,3/100 mil hab.), enquanto a maior tendência de aumento ocorreu na macrorregião Centro-Noroeste, de 2013 a 2018 (VPA = 50,2; IC95% 26,3;78,6). As taxas bayesianas aumentaram na maioria dos municípios. Conclusão: Houve tendência de aumento da sífilis adquirida, principalmente nos dois últimos triênios.

ABSTRACT

Aim: To assess the research gaps identified in a recent mapping review of orthognathic surgery through their evaluation by clinical experts, leading to a clinically relevant list of research gaps. This will guide future investigations of the topic, focusing on the outcomes of blood loss, infection, and relapse. Methods: The Delphi technique will be used to appraise the identified research gaps. The expert panel will include maxillofacial surgeons who regularly perform orthognathic surgery. Potential participants will be identified through various methods, including contact information from articles in the mapping review, nominations from peers, and social media platforms. Two rounds of surveys will be undertaken with Likert-type and open-ended questions to assess the clinical relevance of research gaps. For the second round, participants will receive a report of the results of the first round. Questions will be modified depending on the answers obtained in the first round. A consensus of 60% will be considered valid. Conclusions: Through this Delphi study, in a collaborative effort between researchers and clinical experts, a comprehensive understanding of the clinical relevance of research gaps in orthognathic surgery will be achieved. The outcomes will guide future investigations, ultimately improving the outcomes and practices in this field.

Background: Correct mapping of the blind spot is important, as it serves as an estimate of fixation reliability. When the blind spot is not seen in the expected location in Humphrey visual field (HVF) printout, the clinician should give a thought to why this might be the case. Purpose: This video describes a series of cases, in which due to different reasons the blind spot could not be seen in the presumed expected location in the grayscale and numeric data of the HVF printout and the possible explanation behind this. Synopsis: When interpreting perimetry results, it is important to know whether the field test is reliable or not. A stimulus presented at the location of physiologic blind spot should not be seen by a patient with a steady fixation in Heijl– Krakau method. Responses will also occur, however, if the patient has a tendency for false?positive responses, or when the blind spot of the properly fixing eye is not in the location where the test stimulus is presented, because of anatomic variation, or if the patient's head is tilted while performing the test. Highlights: Perimetrist should recognize these potential artifact, during the test and relocate the blind spot. In case, such results are seen after finishing the test, it is recommended for the clinician to repeat the test

Resumo: Em função da celeridade de transformações no ambiente externo às organizações, se faz necessário realizar adaptações internas dos processos de trabalho e a adesão das tecnologias contemporaneamente ofertadas. Assim, são requeridas dos gestores novas competências, a serem permanentemente desenvolvidas e aprimoradas. O Sistema Único de Saúde, onde o atendimento e as necessidades dos usuários devem se dar de forma eficiente, eficaz e de qualidade, inclui na descentralização o mecanismo de regionalização, hierarquização e participação, que demanda por gestores em saúde pública com competências e responsabilidades adequadas nos vários níveis de complexidade. Desse modo, para execução deste estudo foi adotado referencial teórico de gestão e mapeamento de competências de Hugo Pena Brandão. Este estudo teve por objetivo realizar o mapeamento das competências gerenciais relacionadas à saúde pública, a partir da perspectiva de profissionais que atuam na gestão em saúde. A metodologia aplicada teve abordagem quantitativa descritiva exploratória. A pesquisa foi realizada em três etapas. Na primeira, a aplicação de questionário, construído a partir do documento Marco Regional para as Américas (MRCESP), documento oficial da Organización Panamericana de La Salud (OPS), traduzido e adaptado ao contexto brasileiro. Este instrumento compõe-se de 56 questões ou competências essenciais divididas em seis domínios e foi respondido por 119 participantes de forma completa, na plataforma de pesquisa Pesquisaonline®. Os respondentes atuavam na gestão das 22 Regionais de Saúde da Secretaria Estadual de Saúde do Estado do Paraná, entre os meses de fevereiro a abril de 2020, e as respostas resultaram no Grau de Expressão da Competência no Trabalho. A análise de dados foi realizada com apoio dos softwares Microsoft Excel® e R®. Na segunda etapa foi realizado o mapeamento das lacunas (gaps) a partir das competências avaliadas pelos participantes. A terceira etapa foi realizada com a apuração do Grau de Prioridade de capacitação. Identificaram-se lacunas nos domínios 2 e 6, e, de acordo com a tabela de Grau de Prioridade de Capacitação, que classifica o Grau de Expressão das Competências no Trabalho, os escores foram de Prioridade Nula ou Muito Baixa. Desse modo, como contribuições, a investigação realizada permite o planejamento de estratégias de capacitação, salientando que os saberes aprendidos estão voltados à prática profissional, como a integração de informações, a transformação de conhecimentos e agir contextualizados, impactando na qualidade da assistência desenvolvida nos serviços de saúde.


Abstract: Due to the speed of transformations in the external environment of organizations, it is necessary to carry out internal adaptations of work processes and the adhesion of technologies currently offered. Thus, new knowledge and skills are required from managers, to be permanently developed and improved. The Unified Health System, where care and the needs of users must be provided efficiently, effectively and with quality, includes in decentralization the mechanism of regionalization, hierarchy and participation, which demands public health managers with adequate skills and responsibilities at various levels of complexity. Thus, to carry out this study, Hugo Pena Brandão's theoretical framework for management and mapping of competencies was adopted 22 Regional Health Departments of the State Health Secretariat of the State of Paraná. The applied methodology had a quantitative descriptive exploratory approach. The research was carried out in three stages. In the first, the application of a questionnaire, built from the document Regional Framework for the Americas (MRCESP), an official document of the Organización Panamericana de La Salud (OPS), translated and adapted to the Brazilian context. This consists of 56 questions or essential competences divided into six domains. The instrument was completed by 119 participants, through the Pesquisaonline® research platform, between February and April 2020, resulting in the Degree of Expression of Competence at Work. Data analysis was performed with the support of Microsoft Excel® and R® software. In the second, the mapping of the gaps was carried out from the competences evaluated by the participants. In the third stage, the Priority Degree of training was determined. Gaps were found in domains 2 and 6, but they were not as expressive, and, according to the table of Degree of Priority of Training, the answers of the participants, who classify the Degree of Expression of Skills at Work, found scores of Null Priority or Very Low. Thus, as contributions, the investigation carried out allows the planning of training strategies, emphasizing that the knowledge learned is focused on professional practice, such as the integration of information, the transformation of knowledge and contextualized actions, impacting the quality of care provided in the services of health.


Resumen: Debido a la velocidad de las transformaciones en el entorno externo de las organizaciones, es necesario realizar adaptaciones internas de los procesos de trabajo y la adhesión de las tecnologías que se ofrecen actualmente. Por lo tanto, se requieren nuevos conocimientos y habilidades de los gerentes, para ser desarrollados y mejorados permanentemente. El Sistema Único de Salud, donde la atención y las necesidades de los usuarios deben ser brindadas de manera eficiente, eficaz y con calidad, incluye en la descentralización el mecanismo de regionalización, jerarquización y participación, lo que demanda gestores de salud pública con competencias y responsabilidades adecuadas en los distintos niveles de complejidad. Así, para la realización de este estudio, se adoptó el referencial teórico de gestión y mapeo de competencias de Hugo Pena Brandão 22 Departamentos Regionales de Salud de la Secretaría de Estado de Salud del Estado de Paraná. La metodología aplicada tuvo un enfoque exploratorio descriptivo cuantitativo. La investigación se llevó a cabo en tres etapas. En el primero, la aplicación de un cuestionario, construido a partir del documento Marco Regional para las Américas, documento oficial de la Organización Panamericana de La Salud (OPS), traducido y adaptado al contexto brasileño. Este consta de 56 preguntas o competencias esenciales divididas en seis dominios. El instrumento fue diligenciado por 119 participantes, a través de la plataforma de investigación Pesquisaonline®, entre febrero y abril de 2020, dando como resultado el Grado de Expresión de la Competencia en el Trabajo. El análisis de datos se realizó con el apoyo del software Microsoft Excel® y R®. En el segundo, se realizó el mapeo de las brechas (gaps) a partir de las competencias evaluadas por los participantes. En la tercera etapa se determinó el Grado de Prioridad de la formación. Se encontraron brechas en los dominios 2 y 6, según la tabla Grado de Prioridad de la Formación, que clasifica el Grado de Expresión de las Habilidades en el Trabajo, los puntajes fueron de Prioridad Nula o Muy Baja. Así, como aportes, la investigación realizada permite planificar estrategias de formación, destacando que los conocimientos aprendidos se focalicen en la práctica profesional, como la integración de la información, la transformación del saber y las acciones contextualizadas, impactando en la calidad de la atención brindada en la servicios de salud.

ABSTRACT

The properties of dry-coated paracetamol particles(fast-dissolving model drug)with carnauba wax particles as the coating agent(dissolution retardant)were investigated.Raman mapping technique was used to non-destructively examine the thickness and homogeneity of coated particles.The results showed that the wax existed in two forms on the surface of the paracetamol particles,forming a porous coating layer:i)whole wax particles on the surface of paracetamol and glued together with other wax surface particles,and ii)deformed wax particles spread on the surface.Regardless of the final particle size fraction(between 100 and 800 μm),the coating thickness had high variability,with average thickness of 5.9±4.2 μm.The ability of carnauba wax to decrease the dissolution rate of paracetamol was confirmed by dissolution of powder and tablet formulations.The dissolution was slower for larger coated particles.Tableting further reduced the dissolution rate,clearly indicating the impact of subsequent formulation processes on the final quality of the product.

ABSTRACT

Polysaccharides exhibit multiple pharmacological activities which are closely related to their structural features.Therefore,quantitatively quality control of polysaccharides based on their chemical charac-teristics is important for their application in biomedical and functional food sciences.However,poly-saccharides are mixed macromolecular compounds that are difficult to isolate and lack standards,making them challenging to quantify directly.In this study,we proposed an improved saccharide mapping method based on the release of specific oligosaccharides for the assessment of Hericium eri-naceus polysaccharides from laboratory cultured and different regions of China.Briefly,a polysaccharide from H.erinaceus was digested by β-(1-3)-glucanase,and the released specific oligosaccharides were labeled with 8-aminopyrene-1,3,6-trisulfonic-acid(APTS)and separated by using micellar electrokinetic chromatography(MEKC)coupled with laser induced fluorescence(LIF),and quantitatively estimated.MEKC presented higher resolution compared to polysaccharide analysis using carbohydrate gel elec-trophoresis(PACE),and provided great peak capacity between oligosaccharides with polymerization degree of 2(DP2)and polymerization degree of 6(DP6)in a dextran ladder separation.The results of high performance size exclusion chromatography coupled with multi-angle laser light scattering and refractive index detector(HPSEC-MALLS-RI)showed that 12 h was sufficient for complete digestion of polysaccharides from H.erinaceus.Laminaritriose(DP3)was used as an internal standard for quantifi-cation of all the oligosaccharides.The calibration curve for DP3 showed a good linear regression(R2>0.9988).The limit of detection(LOD)and limit of quantification(LOQ)values were 0.05 μg/mL and 0.2 μg/mL,respectively.The recovery for DP3 was 87.32(±0.03)%in the three independent injections.To sum up,this proposed method is helpful for improving the quality control of polysaccharides from H.erinaceus as well as other materials.

ABSTRACT

Objective:To investigate the application value of intervention mapping-based health education on therapeutic cognitive perception of pulmonary arterial hypertension patients.Methods:Totally, 60 patients with pulmonary arterial hypertension from January 2018 to January 2021 in Huazhong Fuwai Central Vascular Disease Hospital were assigned to the experimental group and control group according to admission time, with 30 cases in each group. The control group received routine health education, while the experimental group implemented intervention mapping-based health education. The intervention effects were assessed by Beliefs about Medicine Questionnaire-Specific (BMQ-S), Morisky Medication Adherence Scale (MMAS), 6MWT as well as Borg Scale (BS).Results:At 1 month and 3 months after discharge, the scores of MMAS in the experimental group were significantly higher than in the control group[(5.43±1.17) points, (5.57±1.17) points vs (4.60±1.54) points and (4.87±1.41) points], the differences were statistically significant ( t=2.36, 2.10, both P<0.05). At 3 months after discharge, the scores of necessity belief subscale in the experimental group were higher than those in the control group [(22.93±2.63) points vs (21.27±2.73) points], concern belief subscale scores lower than in the control group [(12.40±2.54) points vs (14.13±3.01) points], 6MWT distance in the experimental group higher than in the control group [(532.13±38.51) m vs (507.03±51.13) m], BS scores lower than in the control group [(3.53±1.20) points vs (4.10±0.84) points], the differences were statistically significant ( t values were 2.12-2.41, all P<0.05). Conclusions:Intervention mapping-based health education can improve therapeutic cognitive perception and medication adherence of pulmonary arterial hypertension, it is helpful to promote rehabilitation of patients.

ABSTRACT

ObjectiveTo construct and evaluate the transformation model of the Constitution in Chinese Medicine Questionnaire (CCMQ) to SF-6Dv1 health utility and broaden the applications of CCMQ. MethodsThe data of CCMQ and SF-6Dv1 were collected from 595 participants at baseline, 3 months and 6 months after the comprehensive intervention suitable for the corresponding traditional Chinese medicine (TCM) constitution. The estimation and validation datasets were constructed, and four statistical algorithms including the ordinary least squares (OLS), MM robust regression (MM), censored least absolute deviations (CLAD) and the Tobit model were used to create alternative models. The mean absolute error (MAE), root mean square error (RMSE) and intraclass correlation coefficient (ICC) were used to evaluate the prediction performance of the model. ResultsThe constitution scores of all TCM constitutions by CCMQ was significantly correlated with the SF-6Dv1 health utility value measured at three timepoints; the health utility value of the SF-6Dv1 was positively correlated with gentleness type (r=0.596, r=0.578, r=0.606, all P<0.05) and negatively correlated with eight unbalanced constitutions (r=-0.586~-0.301, all P<0.05). The MM established based on the subscale scores of CCMQ was the optimal mapping model, and the MAE, RMSE, and ICC values were 0.0741, 0.0930 and 0.766, respectively. Gentleness type, qi-deficiency type, phlegm-wetness type, qi-constraint type, and age were the primary factors included in the model. The measured and predicted value of SF-6Dv1 had a moderate positive correlation (r=0.673, r=0.617, P<0.05) and a good consistency as shown by the Bland-Altman plot. ConclusionBy using MM, the CCMQ can be transformed into SF-6Dv1 health utility value for health economics analysis.

ABSTRACT

ObjectiveTo conduct a visualized analysis of the research related to the use of brain-computer interface technology for stroke rehabilitation in the past ten years, and identify and predict the hot spots and hot trends in order to promote the further development of this field. MethodsThe Web of Science Core Collection database was searched for literature related to brain-computer interface technology for stroke rehabilitation from January, 2011 to October, 2022. CiteSpace 5.8.R3 was used to analyze the number of publications, countries, institutions, authors, keywords, co-citations, and grant support. Results and ConclusionA total of 592 papers were included, and the annual number of publications in this field of research showed a rapid growth trend, and the research enthusiasm continued to increase. The United States was in the leading position in this field, with the highest number of cooperative publications and the highest intermediary centrality; China had certain advantages in this field, but still needed to strengthen the exchange and cooperation with other countries/regions. Foreign institutions and authors had formed a network of close cooperative relationships, and formed a high-impact team represented by Niels Birbaumer, Cuntai Guan, Kai Keng Ang, etc.; there were poor cooperative relationships among domestic authors and institutions, and there were geographical restrictions and lack of high-impact academic groups. The keywords "motor imagery" and "recovery" formed ten major clusters and 15 prominent words with high variation rates, showing a trend of diversification in research directions. The study of the efficacy of upper limb motor rehabilitation and central mechanisms has been the hot topics in this field and will continue for some time in the future; the use of lower limb brain-computer interface systems for improving foot drop, gait and balance in stroke patients and the application of multimodal brain-computer interfaces will probably become a hot topic in the future. Finally, the use of brain-computer interface-guided neurofeedback training for cognitive and language rehabilitation in stroke also needs attention.

ABSTRACT

Neurons migrate from their birthplaces to the destinations, and extending axons navigate to their synaptic targets by sensing various extracellular cues in spatiotemporally controlled manners. These evolutionally conserved guidance cues and their receptors regulate multiple aspects of neural development to establish the highly complex nervous system by mediating both short- and long-range cell-cell communications. Neuronal guidance genes (encoding cues, receptors, or downstream signal transducers) are critical not only for development of the nervous system but also for synaptic maintenance, remodeling, and function in the adult brain. One emerging theme is the combinatorial and complementary functions of relatively limited classes of neuronal guidance genes in multiple processes, including neuronal migration, axonal guidance, synaptogenesis, and circuit formation. Importantly, neuronal guidance genes also regulate cell migration and cell-cell communications outside the nervous system. We are just beginning to understand how cells integrate multiple guidance and adhesion signaling inputs to determine overall cellular/subcellular behavior and how aberrant guidance signaling in various cell types contributes to diverse human diseases, ranging from developmental, neuropsychiatric, and neurodegenerative disorders to cancer metastasis. We review classic studies and recent advances in understanding signaling mechanisms of the guidance genes as well as their roles in human diseases. Furthermore, we discuss the remaining challenges and therapeutic potentials of modulating neuronal guidance pathways in neural repair.

Intervention mapping (IM) is a framework for formulating theory-and evidence-based health education projects with participatory approaches from ecological perspectives.The intervention program designed via IM plays a role in reducing the exposure of cancer risk factors,increasing cancer prevention behaviors,and promoting early cancer screening and rehabilitation of cancer patients.This study summarizes the characteristics,implementation steps,and application status of IM in tertiary prevention of cancer,aiming to provide reference for the application of IM in the health education projects for cancer in China.

ABSTRACT

The eloquent cerebral cortices are involved in movement, sensation, speech, vision, and higher cortical functions. Functional magnetic resonance imaging (fMRI) allows the evaluation of brain function, aiding in neurosurgical planning by mapping eloquent cortical areas. Considering the high cost of the hardware involved, the purpose of this work is to present a more affordable, in-house alternative for these studies that can provide adequate results in a clinical setting. We also present some practical information on how to perform these exams. We describe an affordable in-house hardware solution used by an imaging center, and examples of fMRI paradigms used to evaluate motor and language tasks. The fMRI studies show robust activations in eloquent areas consistent with the tasks performed on the exam. Images of post-processed studies illustrate clinical cases. The fMRI have well-established applications, mapping eloquent cortical areas in patients with brain lesions. In the case of surgical planning, it allows the surgeon to maximize the resection area while minimizing sequelae. More affordable hardware can reduce the cost of these exams, making them more accessible to the general public.


O córtex cerebral eloquente está envolvido nas atividades motora, sensação, fala, visão e funções corticais superiores. A ressonância magnética funcional (RMf) permite a avaliação da função cerebral, ajudando no planejamento neurocirúrgico através do mapeamento de áreas corticais eloquentes. Considerando o elevado custo do hardware envolvido, o objetivo deste trabalho é apresentar uma alternativa mais acessível para estes estudos, que possa fornecer resultados adequados em um ambiente clínico. Também apresentamos algumas informações práticas sobre a realização destes exames. Descrevemos uma solução de hardware acessível utilizada por um centro de imagens, e exemplos de paradigmas de RMf usados para avaliar tarefas motoras e relacionadas à fala. Os estudos de RMf mostram ativações em áreas eloquentes, consistentes com as tarefas realizadas no exame com imagens de estudos pós-processados ilustrando casos clínicos. A RMf tem aplicações bem estabelecidas, mapeando áreas corticais eloquentes em pacientes com lesões cerebrais. No caso do planejamento cirúrgico, permite que o cirurgião maximize a área de ressecção enquanto minimiza potenciais sequelas. Equipamentos mais acessíveis podem reduzir o custo destes exames, podendo aumentar a disponibilização ao público em geral.


La corteza cerebral elocuente está implicada en las actividades motoras, la sensibilidad, el habla, la visión y las funciones corticales superiores. La resonancia magnética funcional (RMf) permite la evaluación de la función cerebral, ayudando en la planificación neuroquirúrgica mediante el mapeo de las áreas corticales elocuentes. Teniendo en cuenta el elevado coste del hardware implicado, el objetivo de este artículo es presentar una alternativa más asequible para estos estudios que pueda proporcionar resultados adecuados en un entorno clínico. También presentamos información práctica sobre cómo realizar estos exámenes. Describimos una solución de hardware asequible utilizada por un centro de diagnóstico por imagen, y ejemplos de paradigmas de RMf utilizados para evaluar tareas motoras y relacionadas con el habla. Los estudios de RMf muestran activaciones en áreas elocuentes, coherentes con las tareas realizadas en el examen, con imágenes de estudios postprocesados que ilustran casos clínicos. La RMf tiene aplicaciones bien establecidas en el mapeo de áreas corticales elocuentes en pacientes con lesiones cerebrales. En el caso de la planificación quirúrgica, permite al cirujano maximizar el área de resección minimizando las posibles secuelas. Un equipo más asequible puede reducir el coste de estas exploraciones, aumentando potencialmente su disponibilidad para el público en general.

Abstract: We aimed to investigate and compare the distribution of establishments that sell food near municipal, state, and private schools in the municipality of São Paulo, Brazil. This cross-sectional, exploratory, and census study was conducted in 3,121 schools. Circular buffers were traced around schools and concentrations or dispersions of food stores (in absolute numbers and densities) were analyzed. A p-trend was calculated to analyze how food stores density behaved as the buffer radius distance increased. Stratified regression models were built to analyze the characteristics of the food environment. Snack bars and street vendors are the most common types of establishments surrounding schools. Some categories of food stores are concentrated (such as candy stores around municipal and private schools, mini markets around municipal schools, and snack bars around private schools), whereas others (such as super and hypermarkets and fruit and vegetable stores) are dispersed around public schools. The food environment around schools shows differences regarding the instance that administers them and private schools have more food stores around them. Poor-quality food environment around schools exposes students to risk factors regarding excessive unhealthy food consumption.


Resumo: Este estudo transversal, exploratório e censitário realizado em 3.121 escolas buscou investigar e comparar a distribuição de estabelecimentos de venda de alimentos no entorno de escolas municipais, estaduais e particulares da cidade de São Paulo, Brasil. Foram traçados buffers circulares ao redor das escolas e analisadas as concentrações ou dispersões de estoques de alimentos (em números absolutos e densidades). Foi calculado o valor de p de tendencia para analisar o comportamento das densidades dos estabelecimentos com o aumento da distância do raio do buffer. As características do ambiente alimentar foram analisadas por modelos de regressão estratificada. Lanchonetes e vendedores ambulantes são os tipos de estabelecimentos mais presentes nos entornos das escolas. Algumas categorias de lojas de alimentos estão concentradas (como lojas de doces em torno de escolas municipais e particulares, minimercados em torno de escolas municipais e lanchonetes em torno de escolas particulares), e outras (como super e hipermercados e hortifrútis) estão dispersas em torno de escolas públicas. O ambiente alimentar ao redor das escolas difere de acordo com a instância que as administra, e as particulares têm mais lojas de alimentos nos arredores. O ambiente alimentar de baixa qualidade ao redor das escolas expõe os alunos a fatores de risco para o consumo excessivo de alimentos não saudáveis.

Se trazaron buffers circulares alrededor de las escuelas y se analizaron las concentraciones o dispersiones de las existencias de alimentos (en números absolutos y densidades). Se calculó el valor de p de la tendencia para analizar el comportamiento de las densidades de los establecimientos al aumentar la distancia al radio del buffer. Las características del entorno alimentario se analizaron por medio de modelos de regresión estratificada. Las cafeterías y los vendedores ambulantes son los tipos de establecimientos más presentes en el entorno de las escuelas. Algunas categorías de tiendas de alimentos están concentradas (como tiendas de dulces alrededor de las escuelas municipales y privadas, minimercados alrededor de escuelas municipales y cafeterías alrededor de escuelas privadas), y otras (como super e hipermercados y tiendas de frutas y hortalizas) están dispersas alrededor de las escuelas públicas. El entorno alimentario alrededor de las escuelas difiere según la instancia que las administra, y hay más tiendas de alimentos en las inmediaciones de las escuelas privadas. El entorno alimentario de baja calidad alrededor de las escuelas expone a los estudiantes a factores de riesgo para el consumo excesivo de alimentos poco saludables.

Introdução: Os gliomas são tumores encefálicos e da medula espinhal que se originam nas células gliais e cuja progressão invade o tecido cerebral adjacentee e dentre eles um dos mais desafiadores são localizados no lobo cerebral da ínsula. Estas artérias irrigam estruturas nobres e sua lesão pode produzir danos sensitivos, motores e de linguagem. Objetivo: Descrever o impacto da extensão da ressecção, sobrevida global e dos resultados funcionais após a cirurgia dos gliomas insulares, quando estes tumores estejam ou não envolvidos pelas artérias lenticuloestriadas. Métodos: Revisão integrativa nas plataformas virtuais em português e inglês, buscando AND ou OR dados através dos seguintes descritores "Gliomas da ínsula, Mapeamento cerebral, Artérias lenticuloestriadas". A busca inicial foi baseada no título e/ou resumo. Decididos os trabalhos incluíveis foi realizada a leitura na íntegra dos textos. Ao total foram estudados 55 artigos. Resultados: O lobo da ínsula fica "escondido" pela sobreposição dos lobos frontal, parietal e temporal. Para alcançá-lo pode-se realizar as abordagens transsilviana ou transcortical. Ocorre que ao chegar na ínsula visualiza-se significativa ramificação constituída pelas artérias lenticuloestriadas, cuja manipulação pode determinar déficit neurológico e, ao se aprofundar no córtex insular, depara-se com outras estruturas tão importantes quanto a própria ínsula. O conhecimento anatômico das artérias lenticuloestriadas e suas relações é de fundamental importância para a ressecção de glioma insular, pois o comprometimento delas e da artéria cerebral média podem determinar a isquemia dos núcleos da base e da cápsula interna. Conclusão: O tratamento dos gliomas insulares permanece como grande desafio. Devido à sua localização e possibilidade de desenvolvimento de déficits neurológicos na manipulação cirúrgica é necessário não somente conhecer sua localização topográfica, mas também a íntima relação vascular com as artérias lenticuloestriadas. O envolvimento delas pelo tumor possui implicações na sobrevida e na preservação da função neurológica. O conhecimento detalhado da anatomia da região é fundamental para diminuir complicações que afetem grandemente a qualidade de vida dos pacientes.


Introduction: Gliomas are brain and spinal cord tumors that originate in glial cells and whose progression invades the adjacent brain tissue and among them one of the most challenging are located in the cerebral lobe of the insula. These arteries supply noble structures and their damage can cause sensory, motor and language damage. Objective: To describe the impact of the extent of resection, overall survival and functional results after surgery for insular gliomas, when these tumors are or are not involved by lenticulostriate arteries. Methods: Integrative review on virtual platforms in Portuguese and English, searching for AND or OR data using the following descriptors "Insula gliomas, Brain mapping, Lenticulostriate arteries". The initial search was based on the title and/or abstract. Once the included works were decided, the texts were read in full. In total, 55 articles were studied. Results: The insula lobe is "hidden" by the overlap of the frontal, parietal and temporal lobes. To achieve this, transsylvian or transcortical approaches can be performed. It turns out that when arriving at the insula, a significant branch made up of lenticulostriate arteries is seen, the manipulation of which can cause neurological deficits and, when going deeper into the insular cortex, one comes across other structures as important as the insula itself. Anatomical knowledge of the lenticulostriate arteries and their relationships is of fundamental importance for the resection of insular glioma, as their involvement and that of the middle cerebral artery can determine ischemia of the basal ganglia and internal capsule. Conclusion: The treatment of insular gliomas remains a major challenge. Due to its location and the possibility of developing neurological deficits during surgical manipulation, it is necessary not only to know its topographic location, but also the intimate vascular relationship with the lenticulostriate arteries. Their involvement by the tumor has implications for survival and preservation of neurological function. Detailed knowledge of the region's anatomy is essential to reduce complications that greatly affect patients' quality of life.

ABSTRACT

ABSTRACT OBJECTIVE To describe the process and epidemiological implications of georeferencing in EpiFloripa Aging samples (2009-2019). METHOD The EpiFloripa Aging Cohort Study sought to investigate and monitor the living and health conditions of the older adult population (≥ 60) of Florianópolis in three study waves (2009/2010, 2013/2014, 2017/2019). With an automatic geocoding tool, the residential addresses were spatialized, allowing to investigate the effect of the georeferencing sample losses regarding 19 variables, evaluated in the three waves. The influence of different neighborhood definitions (census tracts, Euclidean buffers, and buffers across the street network) was examined in the results of seven variables: area, income, residential density, mixed land use, connectivity, health unit count, and public open space count. Pearson's correlation coefficients were calculated to evaluate the differences between neighborhood definitions according to three variables: contextual income, residential density, and land use diversity. RESULT The losses imposed by geocoding (6%, n = 240) caused no statistically significant difference between the total sample and the geocoded sample. The analysis of the study variables suggests that the geocoding process may have included a higher proportion of participants with better income, education, and living conditions. The correlation coefficients showed little correspondence between measures calculated by the three neighborhood definitions (r = 0.37-0.54). The statistical difference between the variables calculated by buffers and census tracts highlights limitations in their use in the description of geospatial attributes. CONCLUSION Despite the challenges related to geocoding, such as inconsistencies in addresses, adequate correction and verification mechanisms provided a high rate of assignment of geographic coordinates, the findings suggest that adopting buffers, favored by geocoding, represents a potential for spatial epidemiological analyses by improving the representation of environmental attributes and the understanding of health outcomes.


RESUMO OBJETIVO Descrever o processo e as implicações epidemiológicas do georreferenciamento nas amostras do EpiFloripa Idoso (2009-2019). MÉTODO O estudo de coorte EpiFloripa Idoso buscou investigar e acompanhar as condições de vida e saúde da população idosa (≥ 60) de Florianópolis em três ondas de estudo (2009/2010, 2013/2014, 2017/2019). Com uma ferramenta de geocodificação automática, os endereços residenciais foram espacializados, permitindo a investigação do efeito das perdas amostrais do georreferenciamento em relação a 19 variáveis, avaliadas nas três ondas. A influência de diferentes definições de vizinhança (setores censitários, buffers euclidianos e buffers pela rede de ruas) foi examinada nos resultados de sete variáveis: área, renda, densidade residencial, uso misto do solo, conectividade, contagem de unidades de saúde, e contagem de espaços livres públicos. Coeficientes de correlação de Pearson foram calculados para avaliar as diferenças entre as definições de vizinhança de acordo com três variáveis: renda contextual, densidade residencial e diversidade de uso do solo. RESULTADO As perdas impostas pela geocodificação (6%, n = 240) não ocasionaram diferença estatística significativa entre a amostra total e a georreferenciada. A análise das variáveis do estudo sugere que o processo de geocodificação pode ter incluído uma maior proporção de participantes com melhor nível de renda, escolaridade e condições de vida. Os coeficientes de correlação evidenciaram pouca correspondência entre medidas calculadas pelas três definições de vizinhança (r = 0,37-0,54). A diferença estatística entre as variáveis calculadas por buffers e setores censitários ressalta limitações no uso destes na descrição dos atributos geoespaciais. CONCLUSÃO Apesar dos desafios relacionados à geocodificação, como inconsistências nos endereços, adequados mecanismos de correção e verificação propiciaram elevada taxa de atribuição de coordenadas geográficas. Os achados sugerem que a adoção de buffers, favorecida pela geocodificação, representa uma potencialidade para análises epidemiológicas espaciais ao aprimorar a representação dos atributos do ambiente e a compreensão dos desfechos de saúde.

ABSTRACT Objective This study aims to build geographic models related to the nutritional status of adolescents and describe territories regarding the prevalence of malnutrition, overweight, and obesity, in order to spatially represent how the nutritional status of adolescents is distributed in the city. Methods Using geocoding techniques, graphic models were built using data from the SISVAN platform, as well as the addresses and nutritional status of adolescents aged 10 to 19 years in the municipality of Divinópolis, in the state of Minas Gerais (Brazil), between 2020 and 2021. Results There was a prevalence of 34% of obesity and overweight in the 2020 and 2021 samples. The graphical models showed that there is no specific pattern of points for the spread of nutritional diagnoses, but it was possible to identify areas of heat and places with a higher concentration of overweight. Underweight had a homogeneous spread and did not stand out in the formation of profiles. Conclusion Geographic tools with the adolescents' nutritional profile were successfully modeled, which have the potential to contribute to better health indicator management in the assessed territory, even with the limitations of the study.


RESUMO Objetivo Este trabalho tem como objetivo construir modelos geográficos relativos ao perfil nutricional de adolescentes e descrever territórios quanto à prevalência de déficit de peso e excesso de peso, de modo a representar espacialmente como o diagnóstico nutricional de adolescentes está distribuído na área estudada. Métodos Através da técnica de geocodificação foram construídos modelos gráficos utilizando a plataforma SISVAN, os endereços e o estado nutricional de adolescentes de 10 a 19 anos do município de Divinópolis, Minas Gerais, nos anos de 2020 e 2021. Resultados Houve prevalência de 34% de excesso de peso na amostra de 2020 e 2021. Os modelos gráficos mostraram que não há um padrão específico de pontos de propagação dos diagnósticos nutricionais, porém foi possível identificar áreas de calor e locais de maior concentração de excesso de peso. O déficit de peso teve um espalhamento homogêneo e não se destacou na formação de perfis. Conclusão Foi possível modelar ferramentas geográficas com o perfil nutricional dos adolescentes, as quais têm potencial de contribuir para a melhor gestão de indicadores de saúde no território avaliado, mesmo com as limitações do estudo.

Resumo Introdução: O suicídio é um grave problema de saúde pública, ainda permeado de estigmas, fato que dificulta o registro desses eventos e o cuidado às pessoas envolvidas. Por se tratar de um acontecimento multicausal, a construção do cuidado perpassa por ações intersetoriais. Objetivo: Cartografar os elementos envolvidos no contexto de vida de uma mulher que tentou suicídio (usuária-guia), destacando os caminhos percorridos no processo de subjetivação e na produção de cuidado no seu território existencial. Método: Foram realizadas entrevistas semiestruturadas com a própria usuária-guia, amigos, familiares e profissionais de saúde. Esta pesquisa foi realizada no município de Icapuí-CE de agosto/2018 a março/2019. A análise baseou-se em conceitos da Filosofia da Diferença, da teoria de Michel Foucault e da Saúde Coletiva. Resultados: Os caminhos trilhados pela usuária-guia formaram seu território existencial, nesse processo de territorialização, desterritorialização e reterritorialização. Percebeu-se que a rede social foi sua maior proteção, mas também sua maior vulnerabilidade, perpassando por estigmas e identificações. Conclusões: Considerando o despreparo dos profissionais de saúde e da comunidade em lidar com o suicídio, aposta-se em programas permanentes e intersetoriais de prevenção, capacitação e amparo às pessoas envolvidas nesse contexto, além de práticas interprofissionais, principalmente na Atenção Primária à Saúde.


Abstract Background: Suicide is a serious public health problem, still permeated by stigma, a fact that makes it difficult to record these events and care for the people involved. As it is a multicausal event, the construction of care goes through intersectoral actions. Objective: To map the elements involved in the life context of a woman who attempted suicide (guide user), highlighting the paths taken in the subjectivation process and in the production of care in her existential territory. Method: Semi-structured interviews were conducted with the guide user, her friends, family, and health professionals. This research was carried out in the city of Icapuí,Ceará, from August/2018 to March/2019. The analysis was based on concepts of Philosophy of Difference, Michel Foucault's theory, and Collective Health. Results: The paths taken by the guide user formed her existential territory in this process of territorialization, deterritorialization, and reterritorialization. It was noticed that the social network was her greatest protection, but also her greatest vulnerability, crossing stigmas and identifications. Conclusions: Considering the unpreparedness of health professionals and the community in dealing with suicide, we focus on permanent and intersectoral prevention, training and protection programs for people involved in this context, as well as interprofessional practices, especially in Primary Health Care.

ABSTRACT Objective: To analyze the temporal and spatial distribution of polio vaccine coverage in Brazilian states. Methods: An ecological time series study was conducted using data from the National Immunization Program Information System. The analyzed period was from 1997 to 2021. Joinpoint software was used to calculate the annual percentage change and average annual percentage change through regressions. QGIS 3.10.7 software was used to construct thematic maps. GeoDa 1.20.0.10 software was used to estimate spatial autocorrelation using the Global Moran's Index and Local Moran's Index. Results: National vaccine coverage in 1997 was 89.27%, decreasing to 61.32% in 2021. The trend analysis indicated an average annual decrease of 1.5% in polio vaccine coverage in Brazil. Across the country, 17 states showed a statistically significant reduction in the average annual percentage change rate. The highest average reduction rates in vaccine coverage among Brazilian states were observed in Amapá (−3.7%; 95%CI −6.0; −1.4) and Pernambuco (−3.3%; 95%CI −4.0; −2.5). In the spatial analysis, in Moran Global, a positive autocorrelation was identified in the years 2012 to 2021 (p<0.02), with an index value of 0.361, which means that geographically close areas tended to have similar levels of vaccination coverage. Conclusion: There was significant heterogeneity in coverage among states and a strong decrease trend in vaccination rates, which could facilitate the circulation of the poliovirus and pose a threat to the susceptible population.


RESUMO Objetivo: Analisar a distribuição temporal e espacial da cobertura da vacina contra poliomielite nos estados brasileiros. Métodos: Estudo ecológico de séries temporais, cuja fonte de dados foi o Sistema de Informação do Programa Nacional de Imunizações. O período analisado foi de 1997 a 2021. Utilizou-se o software Joinpoint para calcular a variação percentual anual e variação percentual anual média por meio de regressões. Para construção de mapas temáticos foi utilizado o software QGis 3.10.7. Para estimar a autocorrelação espacial com o Índice de Moran Global e Índice de Moran Local foi utilizado o software GeoDa 1.20.0.10. Resultados: A cobertura vacinal nacional em 1997 foi de 89,27%, passando para 61,32% em 2021. A análise de tendência apontou o decréscimo médio de 1,5% ao ano na cobertura da vacina contra poliomielite no Brasil. Em todo o país, 17 estados apresentaram redução estatisticamente significativa na taxa de variação percentual anual média. As maiores taxas médias de redução da cobertura vacinal entre os estados brasileiros foram observadas no Amapá (−3,7%; IC95% −6,0; −1,4) e em Pernambuco (−3,3%; IC95% −4,0; −2,5). Na análise espacial, no Moran Global, foi identificada autocorrelação positiva nos anos de 2012 a 2021 (p<0,02), com valor de índice de 0,361, o que significa que as áreas geograficamente próximas tenderam a ter níveis semelhantes de cobertura vacinal. Conclusão: Evidenciou-se expressiva heterogeneidade na cobertura entre os estados e forte tendência de queda dos índices, o que pode propiciar a circulação do poliovírus e colocar sob ameaça a população susceptível.
